Description
Hard powered off (because it didn't soft power off) the server, SAS switches, and disk enclosures involved in the system from bug 1343.
Returned power to all, booted server. Server was up but slow-responding for a bit, then eventually kernel panicked.
On reboot, attached log is what was in dmesg, suspect system about to panic.

The code that caused this was backed out in 4013. See also the fix (in illumos-joyent), proving that this code was to blame, at https://github.com/joyent/illumos-joyent/commit/59a2f91c88b8ace7906e853b6359d07495d63096#usr/src/uts/common/io/scsi/adapters/mpt_sas/mptsas.c.
