The signalling mechanism used for the quiesce and sync threads seems needlessly complex.

It could be changed to have the quiesce thread perform all of the coordinating work with open context users, leaving the sync thread to merely wait for txgs to be fed from the quiesce thread.

A positive side effect of changing this is that we can find a better way to keep active scans running.
