Description

This fixes several issues with PV disk devices and does some long needed simplification:

- skip CD xdf devices while doing labeling which fixes the panic and other issues Michal is seeing
- drop (useless, ugly, annoying) pv_cmdk and hvm_cmdk modules, and make cmdk module a stub, allowing to have only xdf devices for "IDE" virtual disks
- drop (same description) pv_sd and hvm_sd modules, and given that we don't fully attach the CD xdf devices, make real sd module attach here
- fixup the target numbers for "IDE" virtual disks in devfsadm, so that first disk is now cXt0d0 and not cXt768d0
- provide hard-coded device inquiry properties (much better than nothing)
